Dr. Nicole Durrett

PharmD, BCACP, CDCES

Dr. Nicole Durrett, Reliance’s primary pharmacist, has more than fourteen years of clinical experience educating and managing patients with type 2 diabetes, hypertension, dyslipidemia, obesity and other chronic diseases. She is Board Certified in Ambulatory Care and a Certified Diabetes Care and Education Specialist (CDCES) with extensive clinical knowledge and expertise in managing challenging patient cases. She formerly pioneered a Transitional Care Management (TCM) service for a primary care practice and in a hospital-based outpatient pharmacy setting, has conducted Annual Wellness Visits (AWV), Medication Therapy Management (MTM) reviews, and presented continuing education programs for pharmacy and medical staff. She formerly worked in an outpatient pharmacy helping to triage and resolve prescription and medication related issues and has a vast knowledge of pharmacy practice and commonly encountered setbacks in providing patient care.
