Pharmacists have the knowledge and expertise to ask the right questions, identify medication concerns, and provide comprehensive answers.

Legislation is now in place in Florida to allow a Certified Clinical Pharmacist to work with a physician under a Collaborative Pharmacy Practice Agreement (CPPA) to manage certain chronic health conditions that impact patients and your reimbursement. (Statute 465.1865)

HOW IT WORKS

The Physician and Pharmacist will enter into a collaborative practice agreement, according to the agreed upon clinical guidelines, that specifically outlines the patient’s 

  • Specific chronic disease

  • Education to be provided

  • Laboratory monitoring needed

  • Frequency of follow up with the pharmacist and physician

Patients will maintain follow up visits with their primary physician, as needed and as requested by the patient and/or physician. Enhanced disease management services are in addition to the patient’s regular primary care services but intended to allow more time for disease management and focus on patient-specific education needs. 

The Physician will refer patients who meet specific criteria to the pharmacist for enhanced disease management services. The Pharmacist will communicate with the patient’s physician any changes recommended in medication therapy and laboratory testing needed and will document these recommendations in the electronic health record of the patient.

  2. Can this be submitted to insurance?

  3. Are you my employee?

    No, we would be entering into a Business agreement in addition to the state mandated collaborative practice agreement. We are a consultant, and would not require employment status. All fees and payments would be made according to the business agreement.
